Barona Recreation Department Forms

The following forms are available for download. We have two categories for registration Tribal/Community Member and Non-Tribal please see below.

  1. Tribal/Community Member

Participant must meet one of the following criteria’s:  Barona Tribal Member, Barona Tribal Member Dependent, Non-Tribal Barona Resident. Please download the following forms.

  • Registration Packet (register up to 5 members in your household)

  • Blue Emergency Card (One per person)

  • Handbook for you to read and keep

A new registration packet is required for ANY child (new or returning) effective June 14, 2023. Packet must be completed and handed into Recreation Department Office.

2. ​Non-Tribal

Participant must meet one of the following criteria’s: Non-Native American attending Barona Indian Charter School, Native American, Barona Employee Dependent. Please download the following forms.

  • Registration Packet (register up to 5 members in your household)

  • Yellow Emergency Card (One per person)

  • Handbook for you to read and keep
